Job Description
East Lancashire Alliance is seeking a dedicated and skilled Advanced Nurse Practitioner for their Care & Nursing Homes in Rossendale PCN. This permanent position in Bacup, OL13, offers a unique opportunity to provide first-contact medical advice, continuity of care, and advanced clinical nursing practice.
Primary Duties and Responsibilities:
Provide first-contact medical and nursing advice for care homes, utilizing skills in history taking, physical examination, problem-solving, and clinical decision-making to establish diagnoses and management plans. Ensure continuity of care for housebound patients, making use of advanced clinical skills to deliver appropriate and safe care. Collaborate with the multidisciplinary team to provide safe, high-quality care. Conduct clinical nursing practice at an advanced level, utilizing expert skills and critical judgment. Triage and treat patients, perform specialist assessments, and make necessary referrals to primary and secondary health care teams. Coordinate the planning and delivery of holistic care, ensuring patient and carer involvement. If an independent prescriber, prescribe and review medication based on evidence-based practice and protocols. Promote health and well-being through health promotion, health education, screening, and therapeutic communication. Assess, diagnose, and treat patients in practice premises or their own homes, referring to appropriate agencies as needed. Maintain accurate consultation data in patient records according to NMC guidance.
Professional Role:
Work towards obtaining the Advanced digital badge. Stay current with skills and knowledge, maintaining awareness of professional issues at an advanced level. Assess effectiveness of clinical practice and care delivery through self-reflection, peer review, and formal evaluation. Promote evidence-based practice through the latest research-based guidelines and practice-based research development. Participate in continuing professional development to ensure evidence-based knowledge and competence in all aspects of the role. Maintain professional registration and comply with the NMC Code of Professional Conduct. Record accurate consultation data in patient records in accordance with NMC guidelines. Work collaboratively within and outside the PCN. Participate in audits, inspections, and PCN meetings.
Teaching and Mentoring Role:
Foster a learning environment for patients, care home staff, and other health professionals. Contribute to the planning and implementation of teaching for existing staff, including medical and nursing students, GP registrars, and health care assistants.
Health and Safety/Risk Management:
Comply with Health and Safety policies and report incidents using the Incident Reporting System. Adhere to the Data Protection Act and the Access to Health Records Act. Maintain confidentiality of patient information and business-related information. Support equality and diversity policies, ensuring fair treatment for all.
Skills and Qualifications:
Nursing skills with advanced clinical practice. Strong communication and problem-solving abilities. Ability to work as part of a multidisciplinary team. Independent prescriber qualification (if applicable). Commitment to professional development and evidence-based practice.
