About the Role
The Advanced Nurse Practitioner at East Lancashire Alliance is a crucial role focused on providing advanced clinical care within care and nursing homes in the Bacup area. The post-holder will be responsible for offering first-contact advice on medical and nursing conditions, ensuring continuity of care for housebound patients, and delivering high-quality care using expert clinical skills. This role involves working closely with a multi-disciplinary team to manage acute and chronic conditions, promote health and well-being, and ensure safe and appropriate care. The role also includes prescribing and reviewing medication, coordinating care plans, and making necessary referrals to other healthcare professionals. Additionally, the Advanced Nurse Practitioner will contribute to teaching and mentoring healthcare staff within the organization, fostering a learning environment that supports continuous professional development.
Key Responsibilities:
