    Job summary: To work as part of the Acute Older Peoples Mental Health Service, focusing on the direct needs of service users. Delivering individual treatment plans across boundaries of care, working closely with Care Co-ordinators through the care programme approach and care management process.

    Main duties of the job:

    • To provide support, give time to service users within the unit and the community and thus promote recovery. To provide holistic care, taking into account psychosocial and physical healthcare needs of the service user group.
    • To assist Care Co-ordinator or key worker and service user to assess, plan, implement and evaluate individual care plans and undertake other assessments as required.

    Requirements

    • NVQ Level III or Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience.

    About the Role

    The Post Holder must have the individual service users needs at the fore at all times and use the agreed set of values and skills to underpin their day to day work. They will supervise and support junior staff (Band 2) in their roles, providing advice and acting as a role model and mentor. The Post Holder will work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis, and to take charge of an area in the absence of qualified staff, making judgements around service users conditions, at that time, and the requirements of the area.

    They will ensure that service users engage effectively with the agreed care plan and across appropriate services provided on a regular and consistent basis. The Post Holder will plan and supervise individual or group therapy sessions, in line with the agreed care plan or programme of care. They will provide regular and practical support to service users and their carers/significant other in developing and managing dignity and independence. They will provide support and assistance, as necessary, with daily living activities including personal care.

    The Post Holder will be involved with facilitating community and hospital appointments when necessary. They will positively promote independent living of service users within the community. They will work flexibly with Key Worker, Care Co-ordinator, other relevant community support, service user and significant other to implement plan of care. They will be handling service users cash and valuables on a regular basis. They will help identify early signs of relapse by monitoring the services user progress, level of functioning and mental state and alert the appropriate staff involved in the persons care. They will assist the Care Co-ordinator/Key Worker and service user in developing crisis/contingency plans and advanced statements, as per local policy guidelines.

    The Post Holder will maintain adequate records, as required, by existing procedures, entering appropriate details on the service users case notes, as necessary. They will be able to communicate routine information in an arena where there will be barriers to understanding, and to be able to use tact and persuasion in a non aggressive manner. They will provide information on health promotion and wellbeing. They will ensure the service user understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individual. They will attend and participate in agreed training, development and supervision sessions as an individual and as part of the team. They will undertake such other duties as may be determined from time to time within the general scope of the post. They will adhere to health and safety legislation. The Post Holder will be required to have access to a vehicle for business purposes.
