Requirements
- Degree in appropriate health or social care related subject or equivalent knowledge, skills and experience.
- Evidence of consistent pattern of learning from education, training and experience.
- Awareness of Community Care legislation.
- Awareness of Care Act Criteria.
- Assessment of Care Management.
- Process, policies and standards.
- Awareness and experience of Health & Social Care systems.

About the Role The Health and Social Care Coordinator will work within the Integrated Hospital Discharge Team based at Derriford Hospital. The role involves managing the coordination of complex discharges, providing business support to the team, and ensuring adherence to discharge standards according to the Care Act and National Discharge Guidance. The coordinator will also manage the day-to-day recording and verification of delayed transfers of care, support staff training and development, and maintain patient flow from the acute hospital to their pathway destination.
Key Responsibilities
- Provide business support to the Integrated Hospital Discharge Team.
- Manage the coordination of the complex case load knowledge and understanding of caseload pressures according to ward areas.
- Advise and provide support to Discharge Case Managers to ensure they adhere to process and discharge standards according to Care Act and National Discharge Guidance.
- Coordinate all Complex Discharge referrals.
- Manage the day-to-day recording and verification of delayed transfers of care project.
- Manage failed discharges and find appropriate solutions.
- Ensure the delivery of seamless high-quality service to patients and clinical teams.
- Provide managerial and administrative support to the integrated hospital discharge team managers.
- Investigate and answer complaints.
- Support the managers with staff training and development.
- Maintain patient flow from the acute hospital to their pathway destination.
- Triage and coordinate referrals and facilitate the correct discharge pathways.
