About the Role
The Health & Social Care Coordinator will be a key part of the Integrated Hospital Discharge Team at Derriford Hospital. The role involves coordinating complex discharges, providing business support, and ensuring adherence to discharge standards. Responsibilities include managing delayed transfers of care, supporting staff training, and maintaining patient flow from hospital to their destination. Key Responsibilities
- Provide business support to the Integrated Hospital Discharge Team
- Manage coordination of complex caseloads
- Advise and support Discharge Case Managers on process adherence
- Coordinate complex discharge referrals
- Handle day-to-day recording of delayed transfers of care
- Address failed discharges and find solutions
- Ensure high-quality service delivery to patients and clinical teams
- Provide managerial and administrative support
- Investigate and resolve complaints
- Support staff training and development
- Maintain patient flow and coordinate referrals
