About the Role:
The Rehabilitation Support Worker will join the North Somerset Community Beds Therapy team, providing compassionate care and support to service users recently discharged from acute hospital settings. The role involves promoting independence through rehabilitation techniques, completing daily rehabilitation activities under the lead therapists’ guidance, and providing feedback on service users’ progress. The successful candidate will work under direct supervision, ensuring safe, patient-centered, and evidence-based clinical activities within an integrated multi-disciplinary team.
Key Responsibilities:
- Promote optimum independence through rehabilitation techniques
- Complete daily rehabilitation with patients under the lead therapists’ instruction
- Provide feedback on service users’ progress
- Undertake delegated clinical and therapy-led activities
- Deliver health and social care activities, both acute and long-term
- Work within an integrated multi-disciplinary team, including nurses, physiotherapists, and occupational therapists
