About the Role:
The Senior Mental Health Practitioner will manage a caseload and provide care coordination as part of a multi-disciplinary team. The role involves developing therapeutic relationships with young adults experiencing emotional distress and interpersonal difficulties. Responsibilities include case management, care planning, risk assessment, and providing psychological interventions. The role requires regular office duties, clinical and managerial supervision, and participation in team development sessions.
Key Responsibilities:
- Manage a caseload and provide care coordination
- Develop therapeutic relationships and facilitate focused treatment interventions
- Collaborate with service users, carers, and other providers to create effective care plans
- Ensure effective care coordination and communication within care teams
- Conduct risk assessments and manage crisis plans
- Provide psychologically informed assessments and participate in service evaluation

- The role requires regular travel within Leeds, so a full driving licence and car access are essential.
