Requirements
- Good standard of education, including GCSE English and Maths (grades 4-9)
- NVQ or equivalent training/experience in an acute hospital setting
- Ward-based experience in discharge planning
- Demonstrable experience working with a multidisciplinary team (MDT)
- Strong communication skills
- Ability to manage a diverse workload with competing priorities and tight deadlines
- Full clean driving license

About the Role
East Lancashire Hospitals NHS Trust is looking for a proactive and responsive Complex Case Support Worker to join the Integrated Discharge Service. This permanent full-time role involves acting as a central point for ward-level case management of complex cases. The role includes assessing needs, creating discharge plans, and commissioning and reviewing services for safe patient discharge. The successful candidate will work across all ELHT sites, ensuring timely discharge processes for adult patients in acute elective and rehabilitation wards.
Key Responsibilities
- Provide support to the Integrated Discharge Team members
- Establish and maintain productive working relationships with system partners and ward staff
- Work independently, reporting progress and issues to complex case managers
- Handle patient case notes and referral information according to Trust policy
- Communicate with patients, relatives, and healthcare professionals
- Participate in the delayed discharge process and provide updates to ward coordinators
- Advise ward staff on discharge queries and escalate unresolved issues to senior managers
- Organize and prioritize own caseload to meet deadlines and maintain high service standards
- Support compliance with the Hospital and Community Support Policy Operating Model 2021
- Respond to calls promptly and professionally
- Screen and input information on referrals and assessment documents
- Identify risk triggers and initiate appropriate actions
- Utilize electronic systems for patient tracking and referral logs
- Provide additional support, training, and cover for leave and sickness as needed
- Document accurate records and maintain monthly statistics
